Be sure to carry this card with you and hand it to your medical provider in case you have a medical emergency. It can help providers look up your insurance information and bill services to your insurance.

There are a couple of different numbers provided on this card. You will want to pay close attention to these when logging into your account or when you are asked to provide your account information to someone, such as a provider or customer service representative.

  • Your WorldTrips certificate number is for you to log onto Member Portal.
  • Your UnitedHealthcare (UHC) group number allows medical providers to log into the UHC portal.
